Practice Policies & Clinical Guidelines
At Mayflower Behavioral Health, led by Azeezat A. Samuel, PMHNP-BC, we are committed to providing personalized, evidence-based psychiatric evaluations, medication management, and supportive psychotherapy. To ensure an orderly clinical environment, maintain continuity of care, and respect the scheduled time of all patients, we adhere to the following practice policies.
Scope of Outpatient Care
Mayflower Behavioral Health provides outpatient medication management and psychotherapy via telehealth. Our practice is not an acute crisis clinic. Communications sent through the website, email, or patient portal are monitored during standard business hours only.
24-Hour Cancellation Notice
We require a minimum of 24 hours advance notice for any appointment cancellations or rescheduling requests. This allows us to offer the reserved appointment slot to other patients in need of psychiatric care.
Cancellation & No-Show Fees
Late cancellations (less than 24 hours notice) or missed appointments (no-shows) will be subject to the following charges, which are not covered by insurance:
- $100 Fee: Established patient follow-up visits.
- $150 Fee: Initial comprehensive psychiatric evaluations.
Late Arrival Policy
Timeliness ensures comprehensive clinical evaluation. If you join your telehealth session more than 10 minutes late, the visit may need to be rescheduled to protect clinical quality, and late cancellation policies may apply.
Emergency Circumstances
We recognize that medical emergencies and unforeseen severe events occur. Please notify our office as soon as safely possible. Extenuating situations are evaluated with understanding and compassionate clinical discretion.
FMLA, Disability & Legal Evaluations
To maintain the integrity of psychiatric treatment and ensure thorough longitudinal clinical assessment, Mayflower Behavioral Health does not provide single one-off evaluations for employment clearance, legal issues, or disability claims.
Requests for Family and Medical Leave Act (FMLA) documentation, emotional support animal (ESA) letters, or short/long-term disability paperwork are considered strictly on a case-by-case basis only for established patients who have been in continuous, consistent treatment with our provider for an appropriate clinical period. An established treatment plan and dedicated clinical appointment are required.
Care Continuity & Administrative Discharge
Consistent psychiatric follow-up is vital for patient safety, treatment efficacy, and responsible prescribing of psychotropic medications. Repeated missed appointments, chronic late cancellations without communication, or failure to attend required monitoring appointments may disrupt your treatment regimen and can result in administrative discharge from the practice.
In the event of discharge, our clinic will provide up to a 30-day bridge of essential maintenance medications (excluding controlled substances) and referral guidance to facilitate a smooth transfer to another mental healthcare provider.
