Azeezat A. Samuel, PMHNP-BC

Azeezat A. Samuel is a board-certified Psychiatric-Mental Health Nurse Practitioner and the founder of Mayflower Behavioral Health. Her path into mental health care began at the bedside and in the clinic — years of listening to people on their hardest days taught her that the most powerful medicine often begins with feeling truly heard.

She began her training with a Bachelor of Science in Nursing from Chamberlain College of Nursing, then earned her Master of Science in Nursing from Walden University. After graduating, she practiced as a nurse practitioner in an outpatient clinic, where she saw firsthand how much demand there was for mental health care that is accessible, unhurried, and free of stigma.

Today, she is board-certified through the American Nurses Credentialing Center (ANCC) as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC), providing psychiatric and mental health care for adults across Georgia, Washington, Iowa and Arizona state through telehealth.

She advanced her education with a Master of Science in Nursing from Walden University, preparing for advanced practice and psychiatric-mental health specialization.

Clinical practice

Nurse Practitioner, Outpatient Clinic

Practicing in an outpatient clinic, she cared for patients across the lifespan and saw how deeply people need mental health care that listens first.

Board certification

PMHNP-BC — American Nurses Credentialing Center

Azeezat earned national board certification as a Psychiatric-Mental Health Nurse Practitioner through the ANCC — the gold standard for psychiatric advanced practice.
